COVID-19 Release Form
Form of Release for Summer Activities_COVID.pdfDue to the COVID-19 pandemic, the Steamboat Springs School District (District) is offering modified summer activities. Participation in the Districts summer activities is entirely voluntary and at participants sole risk. COVID-19 is extremely contagious and is thought to spread primarily from person-to-person contact. As a result, federal, state, and local governments and health agencies recommend social distancing and, in some locations, have prohibited large gatherings of people. The District is implementing preventative measures intended to reduce the risk of COVID-19 transmission during its activities. However, the District cannot guarantee that a participant will not become infected with COVID-19. Participating in the Districts summer activities could increase a participants risk of contracting COVID-19. Participants shall comply with all District policies, rules, and regulations during summer activities, including those measures implemented by the District to reduce the risk of COVID-19 transmission. Participants who fail to comply with such measures will not be permitted to participate.
Student Activity/Athletic Department Policy
Policy 1617 short.pdfAll students participating in athletics or activities (i.e., dance showcase, school play, etc.) must sign this form. Parent signature is also required. If you filled out the Online Registration, you do not need to fill this out.
Steamboat Springs High School Sports Physical Form
Physical Fom Clinic.pdfAll student athletes must have a current sports physical on file to participate in athletics at Steamboat Springs High School. Please have the physician fill out this form and submit it to the Athletic Office.
Competitor's Medical Form (CHSAA)
Student Eligibility Form.pdfCHSAA requires that all high school athletes and parents sign this form and it must be kept on file at the high school. If you filled out the Online Registration, you do not need to fill this out.
Equipment Waiver
Equipment Waiver.docIf you filled out the Online Registration, you do not need to fill this out. Safety of the student athlete while participating in sports is one of the Steamboat Springs School Districts top priorities. To satisfy that priority, the District provides equipment to its players which it believes to be the most appropriate for the prevention of injury. Should the participating student athlete and the parent or guardian wish to use their own personal equipment, rather than the District-issued equipment, If you filled out the Online Registration, you do not need to fill this out. Steamboat Springs School District cannot be held responsible in the event that the personal equipment does not perform to the same standards as the District-issued one. The participating student athlete and the parent or guardian must understand the risk associated with using personal equipment for practices and games.

Thank you for your interest in volunteering for the 2017 Fall Sports Season. We currently need many volunteers for Football, Boys Soccer, and Volleyball. We depend on parent and community volunteers to make the games possible. Please consider volunteering a few hours so our student athletes can play Football, Soccer, and Volleyball this fall season.
